REFUND POLICY & RETURNED CHEQUES
We at Sigma Sports Solutions understand that unforeseen circumstances may arise after registration which may not permit your child's participation. As such we ask that parents submit in writing a request for refund prior to June 1, 2008. Non-attendance by a participant without such prior notice given and authorized by Sigma Sports Solutions does not constitute a request for refund. Please note that a $50.00 fee will be applied to all refunds. No refund requests will be accepted on or after June 1, 2008.
Note: a $50 fee will be charged for all returned (NSF) cheques.
RELEASE OF LIABILITY, WAIVER OF CLAIMS and PERMISSION TO USE LIKENESS
As a condition of registering for and participating in soccer camp organized and offered by Sigma Sports Solutions, every player and/or his parent or guardian as the case may be must read and sign this release and waiver. By signing this release and waiver, the player and/or his/her parent or guardian signifies that he/she has read this document and fully understands and agrees to its terms. By signing this release and waiver, I agree on my own behalf and on behalf of my child that:
Soccer is a physical activity. I/my child may, by participating, injure myself/himself/herself or trigger or aggravate a medical condition (known or unknown) that I have which may be affected by playing soccer and participating in camp activities. In addition, damage to or loss of my/my child's personal property is a also possibility. I acknowledge and freely accept and assume such possibility and risk and voluntarily release and discharge that I and/or my child have or may in future have against Sigma Sports Solutions, event sponsors, event charities, event volunteers, owners and lessees of the event premises and facilities, and each of their respective predecessors, successors, affiliates, officers, directors, employees and agents (collectively known as Event Organizers) from any and all actions, suits, demands and claims of whatever nature in law, in equity or pursuant to any statute, relating to or arising from any injuries, damage or loss to my person or property, however arising, that I and/or my child may suffer while participating in this event and any related activities.
I also hereby give Sigma Sports Solutions permission for Event Organizers to record any or all of my or my child's participation in this event for photos, motion pictures, TV, radio, recordings, videotapes and other media known and unknown, and to use same, no matter by whom taken, in any manner for publicity, promotion, advertising, trade or commercial purposes without any expectation or right of reimbursements or compensation of any kind.
